To register your child for childcare provision, please complete and return the booking form below. Information will be confidential to the Centre.  These details will be used should your child require emergency medical attention.

The form should be signed (not electronically) by your head of department/manager.

Forms can be returned to [email protected] or sent via the internal post to Bright Beginnings Childcare Centre, Mount Preston Street. If you require a Microsoft Publisher version of the form, please email the Centre.

Please note: the Centre will only open for Childcare if we receive a minimum of 6 requests.
